If you are a benefit-eligible faculty or staff member, complete this form to receive your $25 membership to Capital Bikeshare. The discount is for new annual memberships or when your annual membership ends and cannot be applied to existing annual memberships. The code may be re-issued to another employee if you do not use the code within 60 days. Limit one code per year per employee.
