Optional Life and AD&D Insurance
Employees can elect optional life and AD&D coverage through Lincoln Financial in the benefits enrollment system.
Review rates and employee contributions to determine cost of coverage for optional life and AD&D insurance.
The following optional life insurance options are available:
- Employee: in increments of $10,000, up to a maximum of $1,000,000.
- Spouse or domestic partner: in increments of $5,000, up to a maximum of $500,000 or 50 percent of the employee's coverage (whichever is less).
- Dependent children: in increments of $2,000. Coverage for dependent children is limited to a maximum of $20,000 or 50 percent of the employee's coverage (whichever is less).
Employee optional life must be elected in order to elect optional life coverage for a spouse, domestic partner or children.
Depending on the amount of coverage elected and timing of the election, Evidence of Insurability (EOI) may be required.
The following optional AD&D insurance options are available:
- Employee: in increments of $10,000, up to a maximum of $1,000,000 or five times the employee's annual benefit salary (whichever is less).
- Spouse or domestic partner: in increments of $5,000, up to a maximum of $500,000 or 50 percent of the employee's coverage (whichever is less).
- Dependent children: in increments of $2,000. Coverage for dependent children is limited to a maximum of $20,000 or 50 percent of the employee's coverage (whichever is less).
Employees may elect optional AD&D without electing optional life; however, optional employee AD&D coverage must be elected in order to elect optional AD&D coverage for a spouse, domestic partner or dependent children.
