When Does Coverage End and Proof of Coverage

  • Most coverage for your dependent child will continue until the end at midnight on the last day of the month in which they reach the age of 26. Life and AD&D benefits end on their 26th birthday.
  • As a qualified beneficiary; your eligible dependent will be offered the option of continuing coverage for up to 36 months through COBRA. Official notification will be sent from PayFlex, our COBRA administrator, along with an application and rate information to your W4 address on record, generally within two weeks from the coverage termination date.
  • The COBRA paperwork can be used as an official document to show loss of coverage.

Change Your Health Insurance Enrollment

  • The dependent is removed from your coverage automatically at the end of the month (or on their birthday, in the case of life and AD&D benefits) in which they turn the age of 26. No action is required on your part. If a premium change is required due to a coverage tier change, your payroll deduction is automatically updated. Please review your paystubs to ensure the proper deduction is being taken.
